Exosomes

Basics of
Exosomes in Aesthetics

Exosomes are Eluted (secreted) from virtually every cell in our body and they are specific to that type of cell. For example, Myocardial (heart) cells secrete exosomes that only affect other heart cells and the same is for kidney, liver, muscle, etc. NEO Vital Exosomes are derived from a very specific cell in our body, and that is called a Mesenchymal Stem Cell, which we will refer to as simply a Stem cell. Stem cells also have a singular function in our body and that is to repair and regenerate damaged tissue. (Understand that stem cells repair and regenerate through multiple pathways but today we will focus on just a couple) When we are young our body is teaming with Stem Cells and readily repairs damaged tissue. As we get older, the ability to self-repair becomes more difficult due to a significant decrease in the number of Stem cells in our body. For Example, you probably do not see many teenagers in your clinic or spa for Microneedling or RF therapies to diminish fine lines and wrinkles and to make their skin look younger. When we are young, we can damage our skin (and our body for that matter) and the high population of Stem Cells in our body comes to the rescue. As we age, and the concentration of Stem Cells in our body diminishes, we need some help to recover. Thankfully, regenerative medicine has been on the rise over the last 15 years, and we now have natural innovative products that stimulate our body to heal itself.

Not All Exosomes are Created Equal

The source of the Exosome is very important to determine the kind of result you can have. There are multiple different types of Exosomes that can be used for Aesthetics. Amniotic Fluid derived Exosomes: Amniotic Fluid, in a pregnant woman, is made from epithelial cells so the Exosomes are derived from the epithelial cells. While epithelial cells are excellent at protecting the outside surface of the skin and the lining of our body cavities, they are not specific to regenerating tissue, they are designed to protect the deeper tissue from the outside world. Umbilical Cord Blood/Wharton’s Jelly/Adipose Tissue derived Exosomes: These Exosomes contain approximately 1-3% Stem cells but are primarily comprised of white blood cells and other non-specific cells. These types of Exosomes will have a positive effect on the skin but again, they are not specific to the repair and regeneration of damaged cells. Plant derived Exosomes: (Phyto Exosomes): Plant derived Exosomes contain non-human components, or to put it another way, they have Zero human components and thus have no ability whatsoever to stimulate the repair and regeneration of damaged cells. While you may see some positive results utilizing Plant derived Exosomes, there will be No long term benefits of repair and regeneration of damaged cells. As was mentioned before, NEO Vital Exosomes are derived from 100% Stem Cells, thus have the capability to repair and regenerate damaged cells.

Just a Touch of Science

While we touched on the fact that NEO Vital Exosomes are derived from 100% Stem cells and thus have the capability to repair and regenerate damaged cells, the question you should have is WHY?

There are several things that make NEO Vital Exosomes unique:
  • NEO Vital Exosomes have a laboratory verified amount of protein in each milliliter (ml) of fluid. Why is that important you may ask? Protein is the component in the body that stimulates a cell to change and repair itself. In science, the saying is; “without protein, nothing happens”.
  • NEO Vital Exosomes contain, a lab verified, 500 micrograms (mcg) of protein per milliliter (ml) while other products on the market have been tested to show anywhere from 0 to 3 mcg of protein. That is why you will see some positive results from the other products but it will be much less than what you will see with the protein rich NEO Vital Exosomes.
  • NEO Vital Exosomes have also been tested to prove that the protein was not denatured (damaged) during the processing of the Exosomes. (No other product can prove that).

How it Works

Remember, the NEO Vital Exosomes are derived from Stem cells and the singular function of a Stem cell is to repair and regenerate damaged cells, so the Exosomes take on the same characteristics as the Stem cells. Exosomes are described as a “cargo of biologic material that is wrapped in a bi-layer adipose (fat) membrane”. Stem cell derived Exosomes are attracted to damaged tissue. The damaged cell is like a homing device to Stem cell derived Exosomes so they are attracted to the damaged cells. Once they arrive at the damaged cell, they attach to the cell and the membrane releases its “cargo”. This “cargo” is comprised of:
  • Messenger RNA (mRNA), Micro RNA (miRNA), Growth factors, Cytokines, and Chemokines.
  • The bi-layer adipose membrane is where the multitude of proteins are stored.
  • To get started, the anti-inflammatory Cytokines and Chemokines go to work to balance the environment around the cell while the mRNA goes to work on the damaged cell. The mRNA is what is called a ‘catalyst’, meaning that it stimulates the cell to take action. In this case, to begin the repair process. Since it is a ‘catalyst’, it will continue to stimulate the cell, along with moving to adjacent cells to stimulate them also. Once it has been determined that enough stimulation has taken place, the miRNA will shut down the mRNA and it will stop. This is where the protein comes in. Once the cell has been stimulated to take action, it must have protein in order to affect that change. Without protein, the cell is not capable of repairing itself, no matter how much it is stimulated by the mRNA. The protein provides energy to the cell by stimulating Adenosine Tri-phosphate (ATP), otherwise known as the ‘powerhouse’ for the cell. Once the cell has been stimulated by the mRNA and the ATP pathway is energized with protein, now the cell can actually begin to repair and regenerate itself back to a normal healthy cell. Someone out there is thinking; “what about the Growth Factors, Cytokines and Chemokines”? Of the myriad of growth factors involved in this process, I will only focus on a couple:
  • Fibroblast Growth Factor (FGF): FGF is a family of growth factors that stimulate healthy cells to undergo mitosis and duplicate themselves. Obviously important in the rejuvenation of damaged skin.
  • Vascular Endothelial Growth Factor (VEGF): VEGF is a growth factor that stimulates neo-vascularization or the formation of new blood vessels. This also is incredibly important in creating and maintaining healthy skin.

  • To make it really simple:
  • When the Stem cell derived Exosome releases its “cargo of biologic material” to the damaged cell, the Cytokines and Chemokines act to balance the environment around the cell. (get rid of the toxic inflammatory components)
  • At the same time, the mRNA is stimulating the damaged cell to change.
  • While the protein is giving the cell energy to affect the change.
  • As the cell repairs itself, certain Growth Factors stimulate the healthy cell to duplicate itself while other Growth Factors stimulate the creation of a new blood supply so the healthy cell can sustain the change. This should make it much clearer as to WHY protein is so important to the equation, not to mention the action of the mRNA, miRNA, Growth Factors, Cytokines and Chemokines. There are obviously many more functions that take place, but for the basis of this conversation, this is the most important.

    Size Matters

Exosomes are measured in nanometers, (1 Billionth of a meter or 1 Millionth of a millimeter). An Exosome by definition is being between 30 and 150 nanometers. Let’s put this into perspective; If an average skin pore size ranges from 250 to 500 microns, then several thousand Exosomes could stand shoulder to shoulder and walk through a single skin pore. This means that the Exosomes definitely have the pathway to get to the deeper dermal tissues, and by utilizing the months and months of training that you have endured learning all of the different technologies; you can be certain that these “cargos of biologic material” will get to the damaged dermal tissue and exert their effect.

    Myths

I have heard Plant Exosome salespeople say that, “human exosomes can stimulate the formation of cancer in the body”. This is absolutely false. There are NO clinical studies to show that human derived Exosomes stimulate cancer in the body, in fact, the type of Stem cells that NEO Vital Exosomes are derived from is approved to TREAT certain types of cancer! In addition, there are a multitude of clinical trials currently with the FDA utilizing the use of Stem cells to treat disease. In addition to multiple studies proving the science behind skin and hair follicle regeneration utilizing Stem cell derived Exosomes.
  • There are NO clinical trials utilizing Plant Stem cells or Exosomes to treat disease.
  • There are no clinical studies to show that Plant Stem cells or Exosomes create change in a cell.
  • There are hundreds of studies showing that human derived Stem cells do affect change in a cell.

    Final Thoughts

The source of the product you use during your procedures will determine the extent of outcome you will have with your procedures. Simply put:
  • Cytokines and Chemokines balance the cellular environment.
